Survey Gift-Card [Giveaway Sweepstakes] Official Rules
NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN.
1. SPONSOR This survey sweepstakes ("Sweepstakes") is sponsored by BURST USA, Inc, located at 440 N. Barranca Ave. #4658, Covina, CA. 91723 ("Sponsor").
2. ELIGIBILITY The Sweepstakes is open only to legal residents of the 50 United States, the District of Columbia, Puerto Rico, and U.S. territories and possessions who are 18 years of age or older at the time of entry. Employees, officers, and directors of Sponsor and its affiliated companies, subsidiaries, advertising and promotion agencies, and immediate family members (spouse, parents, siblings, and children) and/or those living in the same household of each are not eligible. Void where prohibited by law.
3. HOW TO ENTER To enter, complete the survey accessible via [insert survey link or instructions] during the Entry Period. Limit one (1) entry per person. Duplicate entries will be void. All entries must be received by 11:59 PM ET on Sunday, August 10, 2025.
4. ENTRY PERIOD The Sweepstakes begins at 12:00 AM ET on Monday, July 21, 2025, and ends at 11:59 PM ET on Sunday, August 10, 2025 ("Entry Period").
5. PRIZE One (1) winner will receive a $500 Amazon.com Gift Card. Approximate Retail Value (ARV): $500.00. Gift card subject to terms and conditions of issuer.
6. WINNER SELECTION AND NOTIFICATION Winner will be selected in a random drawing from all eligible entries received during the Entry Period. The drawing will be conducted on or about Friday, August 15, 2025. The potential winner will be notified via email within 5 days of the drawing. If the potential winner does not respond within 72 hours of notification, the Sponsor may select an alternate winner.
7. ODDS The odds of winning depend on the number of eligible entries received.
8. GENERAL CONDITIONS By participating, entrants agree to abide by these Official Rules and the decisions of the Sponsor, which are final and binding. Sponsor reserves the right to cancel, modify, or suspend the Sweepstakes if fraud or technical failures impair the integrity of the Sweepstakes, as determined by Sponsor in its sole discretion.
9. PRIVACY AND DATA USE Information collected from entrants will be used solely for the purposes of administering this Sweepstakes, verifying eligibility, contacting winners, and improving Sponsor’s services. By entering the Sweepstakes, entrants agree that their survey responses may be analyzed in aggregate form, but no personally identifiable information will be published or shared with third parties, except as necessary to fulfill the prize or as required by law. 10. RELEASE AND LIMITATION OF LIABILITY By entering, entrants release and hold harmless Sponsor, Amazon.com, and their respective affiliates, subsidiaries, employees, officers, and directors from any liability, injury, loss, or damage of any kind arising from or in connection with participation in the Sweepstakes or the acceptance or use of any prize.
